A Voluntary Agreement is your employer’s acceptance of your claim and its agreement to provide you workers’ compensation benefits. This agreement contains important information about your claim and should be issued in every case in which an injured or ill employee receives workers’ compensation payments.
If you receive a Voluntary Agreement, review it carefully; do not hesitate to sign the agreement, if it is in order. After signing it, the District Office will review the agreement before it is approved, in order to insure that your rights are protected.
