Pursuant to Public Act 98-104, enclosed please find Form 98-108 which sets forth the notice that the Workers’ Compensation Commission has established to implement the Public Act. Please utilize this form to notify the dependents of a deceased employee that they may be eligible for death benefits, subject to the filing and eligibility requirements of the Workers’ Compensation Act.
